Registration Information

* A Parent/Guardian is required to bring the following items for registration:

• PROOF OF BIRTH
  (A Birth Certificate, passport, or baptismal certificate)
   
• VERIFICATION OF ADDRESS
  (DEED, Settlement Papers or Lease)
   
• IMMUNIZATION RECORDS
   
• LEGAL PROOF OF CUSTODY
   
• PARENT/GUARDIAN DRIVER'S LICENSE with correct address
   
• PROOF OF RESIDENCE
  (BGE Bill, Cable Bill, Land Line Phone Bill)
